Prior to the New Patient Visit:

We ask that you complete and submit our New Patient Questionnaire and other Forms, at least 2-business days prior to your visit. The questionnaire provides an opportunity for you to thoroughly communicate your (and your partner’s) past medical history. It also allows you to relay, in your own words, any issues or questions you would like to address during your consultation. We appreciate knowing exactly what your priorities are in seeking our assistance.

In addition, we ask that you gather and provide a copy (in advance of your visit), of your medical records to our office. If you do not have a copy on hand, or you’re unable to acquire a copy in time, please complete, and return by fax or email, a signed Record Release Form to our office as soon as possible, and we’ll acquire a copy on your behalf.

The “New Patient” Consultation:

We ask you to arrive 30 minutes prior to your scheduled appointment. This time is needed to complete the registration and check-in process, before you meet with the Physician.

During this visit, you can expect your physician will have reviewed your questionnaire and prior medical records (if provided) in advance, and will be prepared to discuss the issues and/or answer the questions you’ve raised. Your consultation will include a discussion of your medical history, any allergies and medications you are (or have been) taking (both prescription and over-the-counter supplements), along with important lifestyle issues such as exercise, smoking, nutrition, as well as your menstrual and pregnancy history, any past hospitalizations and surgeries, and prior fertility testing and/or treatment. Based on this information, you and your physician will discuss the timing of any recommended tests, along with the treatment options best suited to meet your individual treatment goals.

We understand your time is valuable, and our goal is to make the most of each visit for you. We suggest planning for a minimum of 90 minutes for your initial consultation (more in some cases). Much of this time is spent face-to-face with the physician, but additional time may be needed to have blood drawn and/or a pelvic ultrasound performed. Our staff will explain any of the recommended procedures or tests, as well as when you can expect to receive results.

You’ll be meeting several members of your patient care team, including your physician, a clinical assistant, an administrative assistant, and a financial counselor. Administrative staff are here to assist with medical record requests, appointment scheduling, and other non-clinical questions. We will attempt to verify/confirm your insurance benefits and coverage prior to your visit, and we’ll email you a copy of the information. A financial counselor will be available to review your insurance benefits and provide cost information for the tests and/or treatment option(s) you and your physician have discussed.

Our goal is to answer your questions and provide a path that supports your medical and emotional interests. Most patients leave their new patient visit with a plan of treatment, while others leave with a strategy for completing the test(s) required to get the answers needed to develop a treatment plan at their next consultation. In either case, you should leave our office feeling cared for and clear about your next steps, along with who to contact if you do have questions.
